CAP Services offers free tax preparation services for low- to moderate-income individuals and families who qualify for FREE tax assistance using our dedicated volunteers! Services will be provided in-person.

WONDERING WHAT TO BRING TO YOUR APPOINTMENT?

We’ll provide a packet prior to or during our first meeting that includes the list below and the linked documents. If you’d like to start gathering paperwork or if your mailing hasn’t arrived, see below for what you’ll need to bring to your appointment.

  • IRS intake form with pages 1-3 completed to the best of your ability and Wisconsin intake page 1
  • The original social security card for each person listed on your tax return, including children and/or dependents
  • Your spouse, if you are married or filing jointly
  • Driver’s license or other state- or federally-issued photo ID for all adults at the appointment
  • All W-2s, 1099s and any other documents showing income you received in 2023
  • All expense statements to support adjustments, deductions and credits
  • Form 1095 showing health insurance coverage for prior year returns. If you purchased your insurance through the Marketplace, you MUST bring your 1095-A.
  • Your 2022 tax return, including all the attachments
  • Tax refund worksheet
  • Divorce decree, QDRO and/or notices to allow child deductions, if applicable
  • Self-Employment Worksheet, if applicable
  • Sale of Main Home Worksheet, if applicable

If you misplaced or haven’t received certain items, you may be able to request replacements.

  • Social Security card: Contact your local social security office to complete an application. You cannot apply for a card online. There is no charge for a Social Security card. Find your nearest office by calling 1-800-772-1213.
  • Health Insurance Coverage information: If you and/or your dependents had insurance through the Health Insurance Marketplace, you will receive a form 1095-A in the mail. You can also download this by visiting your Marketplace account at www.healthcare.gov to download a copy of your form.
    If you are employed and purchased insurance through your employer, they will send you a 1095-C. If you have not received one, contact your employer.
  • Income statements: You will receive a W-2 from your employer reporting wages. Contracted workers could receive a 1099-MISC or 1099-NEC.
    Other sources of income are also reported on a 1099 form. These include interest from your bank account (1099-INT), dividends from stock or mutual funds (1099-DIV), proceeds sale of stocks, bonds or mutual funds (1099-B), state or local refunds (1099-G) and pension payments or distributions from an individual retirement account (1099-R). Contact your financial institution directly to obtain replacement copies of the forms.
    If you receive Social Security, SSI and/or SSDI and you have not received your statements or 1099-SSA, you can obtain copies of your Federal statement by calling 1-800-772-1213 and your Wisconsin statements by calling 1-800-362-3002.
  • Expense statements: Homeowners paying mortgage interest will receive a 1098 Form directly from their lender. Contact lender for replacement copy.
    Post-secondary students will receive a 1098-T Form showing tuition and other payments to the school. Most schools place the form in the student’s access online.
    Individuals paying student loans may also receive a 1098-E form showing loan interest paid.
    Family paying for childcare for dependents will receive a Year-End Statement from the provider. If you have not received one, contact your provider for a copy.
    Individuals who make contributions to a retirement account will receive a Form 5497
  • Childcare expenses: Contact your provider if you haven’t received a year-end statement showing the amounts paid for each eligible child.
  • Homestead expenses: Contact your County Treasurer’s office if you have not received a copy of your Property Tax Bill. Renters, contact your landlord(s) to obtain a rent certificate showing how much you paid in rent for the year. If needed, you can download a blank rent certificate.
  • Miscellaneous Information: If you do not have proof of disability letter, you may obtain one from your doctor or from the agency that is paying your disability. It must state the date you became disabled
    If you sold your home, Sale of Main Home Form is included in this mailing and must be filled out.
    If you were self-employed, Self-Employment Worksheet must be filled out.
The following is a list of other common tax forms or other statements you may have received. If you have any of these forms for the current tax year, please bring them to your appointment.

  • Copies of Forms 1040-ES showing estimated tax payments
  • Forms W-2G, gambling income
  • Forms SSA-1099, social security income
  • Forms RRB-1099 and RRB-1099-R, railroad retirement
  • Forms 1099-MISC, non-wage earnings, contract jobs
  • Forms 1099-G, unemployment and/or tax refunds
  • Any other 1099 form you received
  • Records of any other income during 2023, both taxable and nontaxable (for example, alimony, jury duty, SSI, SSD, Wisconsin Works (W2) payments)
  • Records of amounts paid into or taken out of any IRA or other retirement plan
  • Health insurance premiums, employee business expenses, mileage logs, education expenses, childcare expenses (and childcare provider info including tax ID), or other expenses qualifying you for a deduction or credit
  • If you are claiming the Homestead Credit and you or your spouse are under age 62 with no earned income, you will need to provide a statement from the Veteran’s Administration, Social Security Administration, or your physician verifying your disability status.

VOLUNTEER OPPORTUNITIES

Available in Portage, Waupaca and Waushara counties,

Tax Preparers: Assist taxpayers in preparing state and federal returns using TaxSlayer software.

Intake Specialist: Conduct screening to confirm clients have the necessary identification and tax documents for completion of the tax return.
