Work site: 821 E. 1st Ave., Appleton, WI
Start Date: as soon as possible
Starting Pay: $18.32-$19.94 per hour, depending on experience
Status: Full time – 37.5 hours per week

The Katie Beckett Project is a special eligibility process that allows qualifying children with long-term complex needs, living at home, to obtain Wisconsin Medicaid.
Duties: Key responsibilities are to receive referrals and conduct initial eligibility screening, conduct in-home visits, write home visit reports, and be a resource to families in a 23 county region shared with two other Consultants.
BS/BA in nursing or related human services/health related field and two years’ experience working with families or with children with disabilities; or
Two year nursing degree or AA in related human services field and four years of experience; or
High school diploma and six years of related experience
Excellent communication and public relations skills are required
Knowledge of Medicaid and other family support programs is helpful.
