Community 2000 Make Miracles Happen Golf Outing

Help make “Miracles Happen” for families in need in our communities!
Event includes 18 holes with cart, basket raffles, dinner, and live auction.
Schedule:
8 am: Registration, view bucket raffles and auction items
9 am: Shotgun start 18 hole scramble
2 pm (Approx): Dinner, raffle drawings and live auction to follow
$100 per person / $400 per foursome

IMPORTANT INFORMATION:
- Fee includes 18 holes with cart and dinner available at approximately 2:00 pm.
- To register watch for a registration link coming soon or see below if interested in sponsoring the event.
- All participants fee/charges must be paid in full prior to the day of the event (cash, check or credit cards accepted). Make checks payable to CAP Services and send to Community 2000, 1002 South Main Street, Seymour, WI 54165.
- Each team must designate a captain. The team captain will be the only person to check in at registration for the entire team as well as provide any additional information.
- Space is limited.
- Golfers, teams & sponsorships are filled on a first come, first serve basis. Register & submit payment to ensure your place.
- Event held rain or shine.
- Bucket raffles and auction items will be available downstairs/patio area. You do not need to be present to win but all prize(s) must be picked up at our Seymour office within 2 weeks following the event.
- Your participation in the event permits us to take photographs to share with the local newspaper & for use on social media.

- Premier Event Sponsor – $1,500 – Includes: Foursome Team, logo place on marketing materials and social media promotion
- Dinner Sponsor – Please call to discuss options
- Miracle Team Sponsor – $500 – Includes: Foursome Team, one hole sponsor tee & green, one hole contest
- Special Auction Item Sponsor – $250 – Includes: One hole sponsor tee & green, name on the auction item as a sponsor
- Silent Auction/Bucket Raffle Item Sponsor – Donate an item to be used for our bucket raffles, live or silent auction. Items could include golf clubs, weekend getaways, green fees, gift certificates, themed baskets, etc.
- Hole Sponsor, Driving Range Sponsor, Putting Green Sponsor or Beverage Cart Sponsor – $100 each
