Starting Pay: $23.39 – $25.73 per hour, depending on experience
Status: Full-time, 37.5 hours per week

Overview: The Community Health Worker provides health system navigation, care coordination, health education, insurance enrollment assistance, and connections to community resources. Through culturally and linguistically appropriate services, this position reaches participants where they are, reduces barriers, and facilitates access to care that improves health and socioeconomic outcomes.
Duties:
- Using a participant-centered workflow, strengthen coordination between health care providers, community organizations, and individuals to improve health outcomes.
- Conduct outreach to households with an identified need for services, with a particular emphasis on rural communities and individuals disproportionately impacted by the causes and conditions of poverty.
- Complete whole-person assessments to identify participants’ strengths, priorities, and health-related social needs. Work with participants to develop individualized action plans.
- Connect participants to healthcare, public benefits, and community resources that address housing, food security, transportation, financial stability, behavioral health, and other factors influencing health outcomes.
- Provide ongoing support to monitor progress, address emerging barriers, reinforce health education, coordinate services, and adjust action plans as needed. Empower participants with the knowledge and confidence to advocate for themselves.
- With participant consent, communicate referral outcomes, progress, and unresolved barriers to partner providers through a closed-loop referral process that strengthens continuity of care.
- Enter timely documentation of assessments, referrals, service delivery and outcomes in client database. Maintain complete and accurate case files.
- Assist with community presentations related to program services and operations.
Minimum Qualifications:
- Associate degree in health or human services field (such as community engagement, health and wellness promotion, or psychology) plus three or more years of direct service experience, or high school diploma or equivalent plus five years of related experience.
- Certification as a Community Health Worker desired (or provided by agency within one year of hire).
- Moderate proficiency in Office365 Suite within three months of hire.
